Community-based rehabilitation
Program objective
Systematically provide rehabilitation services by developing and using, as much as possible, the personnel and material resources of the local community, with public health centers at the center, for smooth execution of local community health care and health programs for persons with disabilities.
By doing so, promote rehabilitation of persons with disabilities and their social participation so that they can enjoy a healthier life.

Discharged patients who reside in an area under jurisdiction and who were referred to and connected to a public health center Community-Based Rehabilitation (CBR) program from a medical institution

Local community early adaptation program, rehabilitation program, discharge management counseling activities, social participation program, education, and secondary disability management, etc.

Local community rehabilitation council and case management, connection to regional health & medical centers for persons with disabilities, connection to integrated health promotion program, etc.
